Whether your first visit is for a new baby or an older child, please take the time to fill out our new patient forms. We also ask that you bring your child’s current insurance card. This will help us verify your insurance so we can see your child as quickly as possible.
If you are an expecting mother, you don’t need to pre-register with our office. Simply tell the hospital staff that you would like your newborn to be seen in our office within two days of discharge. If your baby is born at another hospital, please call us on the day you are discharged to schedule an appointment for your baby.
